Your First Visit

On your first visit, please bring:

• Your insurance card
• List of allergies
• List of current medications (prescription or over-the-counter) including:
  • Name
  • Dosage
  • Frequency
• Referral from your primary care physician, if required

Appointments

Early morning and early evening office hours are available for your convenience.

Please call as far in advance as possible for an appointment. If you are unable to keep it, please call to cancel so your appointment may be given to another patient.

Insurance Information

Dr. Nierman is a participating physician in most all insurance plans. However, please check your insurance providers list of participating physicians to be sure your program allows you to be seen at our office. Also, please review with your insurance provider the rules and limitations on coverage and reimbursement.

Referrals

If you are insured by a managed care plan, it is your responsibility to obtain your referrals from your primary care physician. This pertains to all visits, including follow-ups.

Prescriptions and Refills

Please have your pharmacy phone number available at the time you call the office with a medical problem or refill.

Fees and Billing

Charges for visits vary depending upon the services rendered.

Co-payments are expected at the time of your visit.

Directions to the Hopedale Street office

From the North: Take route 140 South to Route 16. Turn right at the hospital and at the next traffic light take another right. The office is approximately one mile on the left.

Or you can take 495 south to exit 20 onto Route 85 towards Milford. Turn right off the exit ramp and follow Route 85 to Route 16. Go through downtown Milford, pass the hospital, then take a right at Stone's Furniture -- this is Hopedale St.

From the East:Take Route 16 (Main St.) through downtown Milford. Passing the Hospital, proceed to the next traffic light, turning right at Stone's Furniture onto Hopedale St.

From the West: Follow Route 16 east, turn left at Stone's Furniture onto Hopedale St.

From the South: Take Route 495 to Route 126 -- in Bellingham turn left at the end of the exit ramp. Follow for approximately one mile, turn right at the Mobile gas station onto Route 140 North. Follow to the intersection of Route 16 near the hospital, turn left. Turn right at the next traffic light onto Hopedale St.
